Referring Doctors

We strive to build a long-lasting and mutually beneficial relationship with our referring doctors. To help facilitate the referral relationship, we provide a convenient, electronic referral form that can be completed, printed, and then submitted online along with digital x-rays. This is completely secure and HIPAA compliant.

After you have completed the electronic form, please make sure to Print and then press the Submit button at the bottom to automatically send us the information. The security and privacy of your patient’s data is one of our primary concerns and we have taken every precaution to protect it.

Technical Notes:

• Our online form uses the latest version of Adobe Acrobat Reader. Please download the free plug-in from Adobe’s web site if it is not already installed on your system. It is important that you have at least version 9 of the plug-in to successfully use our online form.
• Mac users may view step-by-step instructions here.
• Chrome users are not able to use the online submission option. Please use another web browser.
